Please use this form to submit your documented family lineage. This information will help to determine the origins and /or relationships of the various surname family lines.

INSTRUCTIONS

It is not necessary to include dates for living persons, nor is it necessary to fill in all the data fields. The date fields may be entered as, dd/mm/yyyy, abt yyyy, range yyyy-yyyy or unknown. Remember to use the maiden name of the female spouse. There are 10 generation fields for your use and if needed a comment field at the end of the form to add additional information. You may use the TAB key to move through the form. DO NOT HIT ENTER until you are ready to submit your data.
